Below is a sample form to use.  I have included some of the more commonly used form elements as examples and for ease of use.  Please delete this instructional text when you have completed your editing.

Please replace the "Form Name" with the appropriate values. 

Form Name


*Text boxes are created by typing a label, in this case "First Name".  The next step is click the "Text Field" button above.  Fill out the information in the window that pops up.

*Radio buttons allow you to limit the answers to one choice.  How this is achevied is by giving all radio buttons the same name, but different option values.  The selected values specifies the values you want pre-selected.

Below you will see an example for obtaining the income level of the user. 

Yearly Income :
$10,000 - $50,000
$51,000 - $100,000
$101,000+

*Check boxes are used when a user can select multiple values for a question.  All checkboxes that are grouped should have the same name.  The selected values will cause the checkbox to be pre-selected for the user

Below is an example for gathering user interests:

Interests:
Hiking
Skiing
Running
Fishing

*Text areas are useful when you want a user to give more information that can comfortably be typed in a text box

*Combo boxes are used when you have list of values for a user to choose from.  This can grow to be a very large number of options.  In the dialog window, you will be asked for text and value.  Text is what the user will see, Value is what will be sent to you in an email if the user chooses that selection.  Set as Selected Value will make an item the default value for the list.   Size will allow more than one line to be displayed at a time, coupled with "Allow Multiple Selects" which can be use to create a List Box.  An example below with a few values to choose from

*Button You must provide an button for the user to submit the form with.  Text(Value) is the label that the user will see on the form, inside the button.  Every form must have a button type of submit, the reset button is optional and will reset the form back to default values.
